Self-Advocates, Advocates, and Family Members,  

If you, a friend, or family member with Intellectual/Developmental Disabilities is involved with the Office of Developmental Programs, and you have concern about your services, lack of services, or information you have been given, please consider using the attached form to express your concerns.  

Be advised: No personal information (Name, address, phone number) will be shared with anyone at ODP. We will take every step possible to ensure that your identity remains confidential. The Co-Chairs of the Self-Advocate and Family Member Advisory Group will receive the filled in Accountability Form and only pass along information that does not identify the individuals involved.

Please help us make the system accountable to individuals living with Intellectual/Developmental Disabilities. Together we can make change happen!
